If you have a missing tooth or several teeth, dental implants are an option you should consider.  A dental implant (mini or traditional) acts as the root of a tooth and allows the attachment of a fixed crown to chew and smile with.

The placement of the OCO Biomedical one piece ISI implants is very straight forward.  No flap and minimal steps to insert; very similar to placing a mini dental implant.  With little surgery very little healing time is required.  Less discomfort is experienced by the patient with this kind of protocol.

A small pilot bit is used to make a small hole in the gum and bone.  I do like in the OCO placement kit the various drill stops.  These are slipped over the pilot bit and will not allow the dentist to drill any further down that the stop. One more item I like in the OCO Biomedical ISI placement kit is the tissue punch.  The rounded end fits precisely into the pilot hole and a perfect plug of tissue is removed the exact size of the dental implant.

The only other step is to use the final sizing bit and the OCO ISI dental implant is inserted into the gum and bone.  Since the dental implant is a one piece design there is no abutment to screw into place.  A impression cap is placed over the OCO ISI implant and off to First Impressions Dental Lab it goes for a porcelain crown.  OCO 4.0 x 10mm

The upper jaw was a bit more challenging in this case.  The bone that had filled in was not very dense and was soft in density.

I decided to place a two piece OCO Biomedical TSI implant.  With this I could place the dental implants and let them heal in the bone without my patient putting undue stress and strain on them.  The thing I like about the OCO TSI is that it has the same thread geometry and immediate stabilization design as the one piece OCI ISI dental implants.  I believe that most cases can be solved with the OCO ISI dental implant….however in those cases where primary stability can not be achieved then the two piece OCO TSI is a great tool to have in the office.

Placement is the exact same protocol. All the step are followed and a cover screw that is included is placed over the OCO TSI dental implant.

Mini Dental Implants have become a popular choice for patients who are missing teeth to replace them with fixed crowns.  This cost effective procedure allows many patients to move forward with dental implant therapy that otherwise would not.  These crowns are just like a crown that would fit over a natural tooth.  The mini dental implant acts as a new root for the crown to attach to.

Thin ridge of bone on the lower jaw where the canine and premolar teeth have been missing for several years.  Without teeth, the ridge of bone will resorb away in height and width.  A mini dental implant will keep that bone from deteriorating and will serve as an anchor for a crown or bridge.

In 30 minutes we had two OCO 3.0mm mini dental implants in place ready to take a impression for First Impressions Dental Labto make two PFM crowns. 

The placement procedure is very simple.  No flap of gum tissue is made.  No real surgery to do.  No fracturing and expanding the bone.  The design and engineering of the OCO Biomedical implants is consistent from the small size of 3.0mm all the way up to 6.0mm in diameter.

They are dual stabilized and in good bone can be used immediately.  The tip of the OCO Biomedical Dental Implants have a bull nose design.  This pulls the bone up and places in under tension not compression.  This important fact promotes bone growth and healing.  Compression (small hole and forcing a larger dental implant into it) creates an environment of bone resorption before healing and stabilization.

My patient came to see me because she was tired of her gum hurting when she would eat where she was missing her upper molar.Occlusal Pre OCO 5.0

I had extracted the molar tooth about 5 months ago due to a failed Root Canal.  We packed the extraction socket with a bone grafting material so her own bone would fill in where the tooth roots used to be. As you can see on the x-ray it filled in very nicely.

I explained to my patient her choices in implants…either two mini implants to support the molar size crown or one regular size implant.  It would cost a bit more to do the traditional implant but that is what she decided on after our treatment planning and consultation.  I measured the space and a OCO ISI One Piece Regular size implant would work just fine here.

 

 

With the OCO ISI dental implant system only 2 burs are necessary to place the implant.  Here we see the pilot bit with a drill stop to keep it from going any farther in the bone than we want.  Its right in line and we proceed to the next step.Slide9

The next size bit is used and a implant replica is placed to verify fit and angle.  All looks good so we gently rotate the OCO ISI one piece dental implant into the gum and bone.

Snap in Denture with OCO Biomedical Mini Dental Implants

It seems like all the patients that need to come see the dentist wait until the very last minute to ask for help.  We really are not that bad here at Lake Norman Mini Dental Implant Center. 

My next patient had a Cone Beam 3D scan done in March of this year.  She had a failing root canal on one of her upper front teeth.  She must talk to some of my other patients because she had done the same thing as several others…..super-glued her post and crown in at least a dozen times.Images[5]  Should I just consider superglue to sell here at the office?

 

All kidding aside…I must say its pretty creative what patients will think up to solve their dental problems.  I know it takes a lot  of courage to come into the dental office.  Those patients who are deathly scared of us please know that we are here to help.  We will not judge.  We only offer solutions and a range of them to fit what your goals are.

On her 3D CBCT we can see the HUGE defect in the bone where the dental abscess is at the tip of the root canal tooth.

And this is from March…its now December…hopefully I could thread in a Intra Lock 2.0mm mini dental implant into the Palatal(roof of the mouth side) bone to hold a temporary crown.

 

 

I took the tooth out preserving as much of the bone as possible.  It was now time to try and get the Intra Lock Mini Dental Implant in.Defect w mini in place

 

 

 

 

 

 

 

The Intra Lock mini dental implant was rock stable (over 40ncm) and now we would pack the defect with some bone grafting material and a collagen membrane.  In a few months we should have good bone to work with.  My initial thoughts are that the mini is going to integrate(heal) into the bone.  If it does not we should have enough new bone to place a new mini implant.

One Piece Solid Regular Implant or 2 Mini Dental Implants to Replace a Missing Upper Molar

My next patient had an upper molar that he wanted to replace.Before oco 4.0 ISI

 

The upper molar had been extracted a few years ago.  The molar behind it had started to tip forward and he really missed it when chewing his favorite food….Prime Rib a J&W Cafeteria.

On the x-ray of the area you can see how the molar behind was starting to tip forward.  You can also see the sinus cavity slowly coming down.  In a few more years I suspect there would be very little height of bone.  Then it would be very difficult to replace his missing molar with a mini dental implant or even a regular conventional implant.

 

 

 

 

 

 

 

One of my biggest objections to using a regular Regular Implant
implant is the two piece design.  Anything that is made of two pieces is never going to be as strong as something that is one solid piece.  Thus a 2.5mm mini dental implant is just as strong as a larger two piece 3.75mm implant. 

I was asked to try out a different kind of regular implant.  An OCO Biomedical solid one piece regular size implant.  To replace this missing molar I would have used 2 Intra Lock Mini Dental Implants from Shatkin FIRST

Since there was not 8mm of height available to place the regular implant I decided to do a sinus lift by tenting the membrane with some Foundation collagen material.  The concept is to carefully penetrate the bone and then widen it just enough to place some of the Foundation material in to push the sinus membrane up and then insert the implant.  Over time the body will fill bone in around the tip of the implant in the sinus cavity.

In the end I think there is a place for either type of implant (regular or mini dental implant).  They both work and they both work well.  It really to me is what the dentist feels works best in his/her hands and what the patients desires are.  Cost certainly is an issue with the mini dental implant being more cost effective.  If I had to to this case over again I would have a hard time choosing the best option for my patient.  Its all on what you like to do and what you are good at.  My final thoughts are that there are so many patients that have missing teeth we should encourage dentists to get trained in placing implants…mini dental implant are a great way to start.

My next patient was going to be a traditional implant case.   The treatment plan he wanted was to replace all his upper missing teeth with regular implants.  This would require a sinus lift on both sides where the lost teeth had allowed the sinus to drop down with no bone left under the gum tissue.

I did the sinus lift on the patients right side first.  When I went over to the left side there was a thin section of tooth root left.  Upon removal of that root there was enough damage to the sinus membrane that I elected to let the sinus heal and would come back at a later date to finish.

Although he healed just fine….the patient elected not to do the sinus lift on the other side and asked if I could change his treatment plan to accommodate a full fixed roundhouse bridge on mini dental implants.   Of course I could…there was good bone where we did the sinus lift and I would not have to expand the bone in the anterior maxilla(front upper jaw).  This is what I believe in…minimal surgery, minimal augmentation of the bone, place what will fit and enough of them to equal the surface area of a regular size implant. 

I had the dental lab produce a wax up model of what I wanted the patient to 'test drive' in his mouth.  A IMG_8061 12 tooth bridge.  I would make this out of chairside arcylic much like a temporary crown that you wear while the dental lab is making the final porcelain crown.

Based off the 3D Cone BeamiCAT scan I decided to place some OCO Biomedical 3.0mm mini dental implants to support the back teeth and OCO Biomedical 2.5mm mini dental implants to support the front teeth. 

Minis Micros Clinical

 

 

OCO Biomedical mini dental implants in place to support a full fixed roundhouse porcelain fused to metal bridge.  This will be like any other bridge that the patient does not remove and will brush and floss like his own teeth.

A standard full arch impression was taken.  The lab will insert the OCO mini dental implant analogs and will make the full roundhouse bridge supported by our mini dental implants.

My patient immediately after the placement of the OCO Biomedical 2.5mm and 3.0 mini dental implants.  He left the office with a totally mini implant supported temporary bridge that was made off the diagnostic wax up.  It was made chairside with Luxatemp temporary material.
