Dental Implants to Replace a Missing Back Molar

Are you missing a back tooth?  Then you should consider getting a dental implant.  A dental implant acts as the root of the tooth and can allow for a crown to be placed over the implant.  No need to grind down healthy teeth on either side of your missing tooth for a bridge.

X ray of my patient showing the missing upper molar tooth.  One option would have been to grind away healthy tooth to create room for a bridge.  The teeth on either side of the missing tooth would have to be drilled on for a 'crown' and that would hold in the bridge.

With a dental implant we can save healthy tooth structure by not having to shape the adjacent natural teeth.  In some cases it can cost less to do a dental implant and crown instead of a bridge.

Clinical picture of the same OCO two piece dental implant with a healing cap.  This is the day of placement.  Notice the lack of blood or stitches or the gum having to be cut and pushed back.  This traditional size dental implant is placed in the same fashion as a mini dental implant.  Very little surgery, very little discomfort and post operative pain for the patients.  Because the bone in this case was very soft I decided to use a regular sized traditional two piece dental implant.

If the bone had been dense I could have placed multiple mini dental implants to support the molar crown as shown in this case.  Both solutions work and work well.  It is really a choice for the patient and dentist on what can be done and at what cost (economic, time, and potential discomfort).

 

 

 

This implant will heal for several months (12-16 weeks) before we can 'load' it with a crown.  In the case shown above with the mini dental implants because there was good dense bone I was able to place the crown over the implants in 3 weeks.  The mini implant crown has been in function now for over a year.

I will continue the post to show the abutment (the part of the implant that comes up out of the gum for the crown to attach to) and final porcelain crown.

Missing Tooth replaced with a Mini Dental Implant and Crown

This a part 2 of the missing lateral tooth post.  After orthodontics some patients have a space from a missing tooth.  My patient Zack was one of those.  His choices were:

Walk around with a plastic retainer with a denture tooth in it.  He would have to remove this retainer every time he wanted to eat or walk around with a gap between his front teeth.

So in one 30 minute appointment Zack had a mini dental implant placed.  A standard crown impression was taken and in 1 week First Impressions Dental Lab returned his final porcelain crown.

Mini dental implants are easy to place, easy to restore, cost effective and a painless way to replace a missing tooth or several missing teeth.

I think all dentists should get educated in placing mini implants.

OCO Mini Dental Implant to Replace a Missing Lateral Tooth with a Porcelain Crown

About 2% of the population has a congenitally missing upper lateral tooth.

Lateral missing

My patient after orthodontic treatment.  He is missing one of his upper lateral teeth.

He is 17 and just wants to enjoy being a kid with teeth.  The solution he has right now is a clear plastic retainer with a denture tooth in it to replace the missing tooth.  His Mom was unable to consider a traditional two piece dental implant due to cost.  In this case I think a one piece dental implant is a better service to the patient and at a much more affordable price point. 

 

 

 

After measuring the space we determined that a OCO 3.0mm Mini Dental Implant could be placed.  It is a one piece immediate load dental implant.

One Piece Dental Implant ~ OCO Biomedical ISI Extraction & Immediate Placement

In keeping with our mission at my dental office we try to make implant dentistry affordable to those patients that have not been able to move forward with their treatment. 

I have been thrilled to find a one piece dental implant in a regular or traditional implant size.  The OCO Biomedical ISI dental implant offers me the option of using a larger implant that does not require additional parts.  The abutment(the part of the implant that comes up out of the gum) is made right along with the body of the dental implant(the part that is inserted into the bone). This offers me the things I like about mini dental implants:

  • One piece design
  • No additional parts to buy
  • No microgap
  • Prepping of the abutment in the mouth is part of the insertion protocol
  • No surgical flap is recommended
  • Less trauma
  • Less healing for the patient
  • Increase in affordability for patients
  • Ease of placement….

My next patient was missing his upper eye tooth and the little tooth next to it was fractured and need to be extracted.Pre op buccal view

Of course there was a lot else going on but this is where we decided to start after getting his periodontal condition as stable as possible. 

 

 

 

Using some PDL-vators from Salvin Dental Supply I removed the tooth with minimal trauma.

The same insertion protocol was used for the OCO One Piece Dental Implant as I would do for a mini dental implant.  Its called logical progression.  If you can place a mini dental implant; add one more bone former and you can put in a one piece regular implant.  I try to follow the philosophy of selecting a implant that will fit the bone NOT make the bone fit the implant with additional surgical procedures.

The same concept is applied to the Mini Dental Implant that will be placed into the extraction site.  It is is one piece dental implant like the OCO ISI 4.0 x 14mm that was placed to act the the root for the missing eye tooth. 

We are going to place a OCO i-mini 3.0 x 16mm to get at a minimum 3mm past the end of the root of the tooth that was extracted.  This is to ensure stability of our mini dental implant for the temporary crown.

With both the traditional one piece dental implant and the mini dental implant in place we can make the temporary crowns to fit over them while they heal.  You can see on the xray that the mini dental implant has gone past the tip of the extraction socket.  The regular size OCO ISI one piece dental implant is firm with over 50ncm of torque.

Two temporary acrylic crowns are made and cemented over the OCO one piece dental implants.  Once the tissue heals in around the extraction site and fills in with bone the final impression will be made for First Impressions Dental Lab to make the Porcelain Crowns over the mini dental implant as well as the regular one piece implant.

My patient who alternates between our office and the periodontist office came in today.  She had been told a few months ago by the periodontist that her upper canine tooth crown was failing and would come out soon. But as most patients and people are we hate to go see our dentist until…..it breaks

To get the rest of the root out without damaging the delicate plate of bone on the cheek side I had ordered some SalvinPDL-elevators.  These are thin bladed instruments used by the dentist to remove the broken root without damaging the bone.  Thanks Chris Totty for sending them out.  I had another brand but needed another set and I quite like these.  Plus Salvin Implant Supply is here in Charlotte NC.Salvin pd-Elevators

Now with the canine root out I could check the quality of bone and pick what dental implant would make the most sense to place.  We could let the area heal and place a few mini dental implants or consider a one piece OCO Biomedical Immediate Stabilized Implant.

Having a stock of OCO Biomedical Implants that Matt Baron and Chris Sanchez had sent me I quickly found a OCO ISI 4.0 x 14mm that would work great in a situation like this.

Following the simple OCO dental implant placement protocol I used the pilot bit to ensure the correct angle was being followed and took an xray to confirm.

I think this case could have been solved either with a mini dental implant capturing the palatal slope of bone or a traditional implant as shown.  I have done both and they both will work well with the proper training.  I am encouraged by the number of dentists who are now seeking to get dental implant education.  There are several great mini dental implant seminars and education as well as regular implant CE courses.  I encourage any dentist to get started and mini dental implants are a great way to move forward in helping our patients.
